Bumbley – A dollop of ice-cream, a piece of chocolate, pizza, macaroni and cheese! Ooh how I love fooood.
Coley – That is not food. That is junk food! And you know that junk food is unhealthy, right?
Bumbley – I know, I know. But why? I mean how can something that tastes so good, be so bad for us?
Coley – It is quite simple Bumbley. We need food to supply us with nutrients. Nutrients which are used to build our body and fuel it.
Bumbley– Why can’t junk food do that for us?
Coley – Because junk food doesn’t have enough nutrients in it. It does not give you energy to play or stay strong.
Bumbley – I don’t mind not playing if I can have an extra burger. (One burger eating another burger and burping)
Coley – Really? No play for a burger?
Bumbley – Actually, I won’t mind both. Play and junk food (Burger trying to balance itself on a ball)
Coley – You can have both. You can have your burger if you eat fruits too. That way you are giving your body enough nourishment too.
Bumbley – I am sleepy now.
Coley – Oh, yes. Junk food makes you sleepy and tired too. You can sleep if you want. I am gonna go and play basketball.
Bumbley – Hey, wait for me! I promise, I will eat fruits and veggies too. Because I wanna play and have fun!

Blood sugar during pregnancy is controlled in 3 ways, one is exercise diet and third is meditation. Usually diet and exercise are the first tried, if the blood sugars do not come under control with these two, then only medications are started. When we come to exercises exercise, as in she needs to go for half an hour to 45 minutes of walking everyday and the speed of the walk it depends on the comfort of the patient and there is something known as isometric exercise . Isometric exercises are done in the sitting posture at home. Here one kg weight are grasped in both the hands and there is flexion of the arm at the elbow joint, flexion and extension are continuous movements about 3 to 4 sets are done each set is topped when the patient is tired and when she has some amount of shortness of breath. These are isometric exercise and any other form of exercise can be performed like yoga she can go for swimming and some amount of light dancing can be done. Coming to diet, diet she has to follow a diabetic diet, she has to take regular frequent meals, she has to keep up her time an these meals are divided into 6 portions, one is breakfast lunch and dinner, and in between breakfast and lunch, and in between lunch and dinner and late midnight snack, here the carbohydrate should constitute about 50 to 55% and proteins about 20 to 25% and fat about 15% and in between snacks should consist about whole-wheat bread and some biscuits devoid of sugar and direct sweets to be avoided and pastries, cakes, chocolates, ice creams to be avoided and sugar has to be brought down, rice content to be brought down, and she needs to eat more of wheat and ragi and after every meal if she can do some amount of light walking, that would very very helpful and the last would be medications. Medications would be oral hypoglycemic agents and insulin. Insulin is given for all diabetic patients. But now oral hypoglycaemic agents like metformin and glipizide are used and the doses of these are adjusted according to the sugar of the patient.

Medical Coding Tips – How to Code Pre Diabetes with Hyperglycemia

Q: How would you code pre-diabetes with hyperglycemia?
A: I’m not going to try to open up Find-A-Code, it will be too slow. If you want to look it up. But we can go to Find-A-Code.

Pre-diabetes is not diabetes, it is not the diabetes code E11. It is not going to be E11, it’s not going to be E10, it’s not going to be E13 or 12. Those are your main endocrine codes, but I can tell you a quick way to look this up. If you know the diabetic codes, you can go into a code like E11.9 and read and it will usually tell you “Hey, heads up. This excludes diabetes” things like that.

You can also look up hyperglycemia and it will usually, the ICD-10 manuals is really good about giving you things to look at and to cross reference, and encoders are even better than the manual because they pop up for you.

Did you have that? Did you find it at all, Schuyler?

Coach Schuyler: I really don’t feel that there’s going to be a combination code because it’s more of the signs and symptoms. It would be more of definitely coding both of them. So you have that pre-diabetes, which is the R73.03. Then, the hypoglycemia is R73.9. Obviously, that’s something that we can more look into, but since it is not a definitive diagnosis, I feel that it would not have that proper combination code like E11.65 does, that diabetes type 2 with hyperglycemia. For the purpose of this, I haven’t found anything that’s what I’ve come into conclusion for in regards to this question.

Coach Alicia: That is a good point to make. That is what separates the people that have experience in coding versus the ones that are newer because you automatically think, Oh, I know. E11 is going to be one of the first codes that you learn because anything you’re going to teach in coding can be done through diabetes, so you’re going to hear it a lot. You’re going to learn I10 for hypertension and E11, so you automatically start thinking of that, but this is actually pre-diabetes and that’s different. Notice that it’s an R code versus an E code which is an endocrine code, it’s how you remember it. It’s an R code which is a completely different area. That should give you a heads-up. It usually confuses everybody so hopefully that’s a good answer for you.

But if you get stumped, that’s the way to do it. Go and look and they’ll say, “Hey, by the way, this excludes pre-diabetes or diabetes due to pancreatectomy or something like that.”
